Templates
Templates in this collection
A practice handles personal and sensitive information every day, so its forms and notices must be clear about what is collected and why. These templates give you plain-worded starting points and leave medical and legal specifics to you and your regulator.
Templates are a careful starting point, not legal advice. Where the law matters, check what applies where you are.
Healthcare practices
Questions people ask
Do these forms replace advice from my regulator?
- No. They are a careful starting point. Health records, consent and data protection rules differ by country and by profession, so check what your regulator and your lawyer require before you use them.
How should completed forms be kept?
- Keep them where only authorised staff can reach them, and for no longer than your rules allow. The data retention policy template helps you write down how long and why.
Are these templates free?
- Most are. Fill in and download them for free as Word or PDF, with no account. A few longer documents are premium: free to fill in, paid once to download, and marked with their price.
Do they follow my country's law?
- They assume no country's law. Where the rules matter, such as safety, data protection or employment, each template says to check what applies where you are, and a lawyer should read anything that carries real risk.
