Job title: Customer Success Manager
Manager: Daniel Ferreira
Reason for absence: Reason for the absence
Absence began: 8 November 2026
Return date: 15 February 2027

1. Purpose

This plan helps Maria Santos return to work safely and with support. It is agreed together, kept private, and can be changed as the person recovers.

2. Advice we have

Fit to return with a phased start. No lifting over 5 kg for four weeks.

3. Duties on return

Week 1 to 2: handle existing accounts only
Week 3 to 4: add new client calls
From week 5: full duties

4. Hours and place of work

Mornings only for two weeks (9:00 to 13:00), then 6 hours a day for two weeks, then normal hours. Home on Wednesdays.

5. Adjustments

A height-adjustable desk
A quiet space for calls
Extra breaks every hour

6. Support

A buddy for the first month
Access to the employee assistance line
Time off for appointments

Pay: Full pay for the first four weeks, as agreed

7. Keeping in touch

Before the return date, Daniel Ferreira and Maria Santos will talk about how the first day will go, what colleagues will be told and what is private. Nothing about health is shared with colleagues unless Maria Santos agrees.

8. Reviews

We will meet on: 22 February, 8 March and 29 March 2027

At each review we check how the person feels, whether the duties and hours still fit, and whether anything needs to change. If the plan is not working, either side can ask for an earlier review.

9. If things get harder

If symptoms return or the work is too much, tell Daniel Ferreira at once. We will look again at the duties, hours and support, and take advice if needed.
