Model Release Form
This release is made on 7 November 2026 between:
- Amira Haddad, of 27 Elm Court, Manchester M1 5AN, United Kingdom ("the Model"); and
- Studio Northlight Ltd, of 3 Foundry Yard, Manchester M4 1QP, United Kingdom ("the Photographer").

1. The shoot
The Model has taken part in a photo or video shoot on 5 February 2027 at Studio Northlight, 3 Foundry Yard, Manchester ("the Shoot"), for the spring 2027 catalogue and website of Fern & Field clothing. In this release, "the Images" means all photographs, video, sound recordings and other recordings made of the Model at the Shoot, in whole or in part, and any edited or changed versions of them.
2. Permission to use the Images
2.1 The Model gives the Photographer permission to use the Model's image, likeness and appearance, as shown in the Images, and the Model's name where it is used in connection with them, and to edit, crop, retouch and combine the Images in the ordinary way.
2.2 The permission applies only to the following media:
websites, social media, print catalogues, brochures, posters, online and print advertising and press material
2.3 The permission applies in this territory: worldwide, and for this period: 5 years from the date of the shoot. After the period the Photographer will stop making new uses of the Images, but is not required to take back what has already been printed or published.
2.4 The permission is not exclusive, and the Model may take part in other shoots. The Photographer may pass the permission to a person who owns or publishes the Images, and to agencies, printers and licensees working for the Photographer, but only for the uses in this release. The Images may not be sold to another person to use for a different purpose without the Model's written consent.
2.5 The Photographer will not use the Images in a way that is offensive or misleading, or that suggests that the Model supports a political party, a religion, a cause, or a product or service other than the one in section 1. The Images will not be used in pornographic or adult material, and the Model will not be shown in a false or damaging way.

The Photographer owns the copyright and all other rights in the Images. The Model has no right to approve how the Images are edited, and gets no share in them, but may ask in writing to see how they are used.
4. Payment

4.2 Fee: GBP 250. The Photographer will pay it within 30 days after the Shoot, unless the parties agree otherwise in writing.
4.3 The Model will receive these copies: 5 edited images within 30 days, for the Model's own portfolio and personal use
5. Withdrawing the permission
5.1 The Model may withdraw this permission for the future by written notice to the Photographer, if the law where the Model lives allows this or if the Photographer agrees. After the notice the Photographer will stop new uses of the Images as soon as it reasonably can, and will take them off its own websites and social media accounts within 30 days.
5.2 Withdrawal does not affect a use that has already happened, material that has already been printed, distributed or published in a way that cannot be called back, or a payment that was made. It does not affect permission to hold the Images for the Photographer's records.
5.3 A Model who has been paid is not required to repay the fee on withdrawing, unless the law or the parties agree otherwise in writing.
6. Privacy
6.1 The Photographer will keep the Model's contact details and any other personal information the Model gave private, will use them only for the Shoot, for payment and for contact about the Images, and will not sell or give them to others, except where the law requires it.
6.2 The Model is informed that the Images may be published and seen by anyone, may be copied by others, and cannot be fully removed once they are on the internet. The Photographer is responsible for its own use and will not itself publish the Model's address or personal contact details with the Images.
6.3 The Model may ask the Photographer in writing for a copy of the personal information that the Photographer holds about the Model, to correct it, and to delete it when it is no longer needed.
7. Promises
7.1 The Model confirms that the Model is at least the age at which a person can sign this release under the law that applies to them, or that a parent or guardian has signed below, and that the Model has the right to give the permission in it and has not made a contract that conflicts with it.
7.2 The Model has read this release, has had the chance to ask questions and to take advice, and signs it freely. The Photographer will treat the Model with respect and will stop the Shoot if the Model asks for a reasonable reason.
8. General
This release is the whole agreement about the Images and the Shoot. It may be changed only in writing signed by both parties. If any part of it cannot be enforced, the rest remains in effect. It may be signed in separate copies and electronically.
9. Governing law and disputes
This release is governed by the laws of England and Wales. The place for deciding any dispute under this release is Manchester. How disputes are decided
